About the Role

Job Summary
The successful candidate will be responsible for assessing patients' physical therapy needs, developing personalized treatment plans, and delivering effective interventions to achieve desired outcomes. You will work closely with patients, families, and other healthcare providers to ensure seamless care coordination and excellent patient outcomes.

Key Responsibilities
- Develop and implement comprehensive treatment plans that address patients' physical therapy needs.
- Provide expert physical therapy interventions, including assessment, evaluation, and consultation services.
- Collaborate with interdisciplinary teams to coordinate patient care and ensure smooth transitions between departments.
- Educate patients and families on self-management techniques and strategies to promote independence and prevent complications.

Requirements
- Bachelor's Degree in Physical Therapy (PT) or equivalent qualifications.
- Current Physical Therapist License in the state where the job is located.
- Strong communication and interpersonal skills, with the ability to work effectively in a fast-paced environment.
