Job Title: Wellness Nurse - Resident Care Specialist
Job Summary: We are seeking a highly skilled and compassionate Wellness Nurse to join our team at Sunrise Senior Living. As a Wellness Nurse, you will be responsible for providing high-quality resident care and services, promoting a safe and healthy environment, and collaborating with our interdisciplinary team to achieve exceptional outcomes.
- Responsibilities:
- Conduct regular wellness visits to assess residents' health and well-being, identify potential health risks, and provide interventions to prevent complications.
- Develop and implement individualized care plans to meet the unique needs of each resident, ensuring their physical, emotional, and social well-being.
- Collaborate with our interdisciplinary team, including the Resident Care Director, to coordinate care and services, including medication management, wound care, and other medical interventions.
- Communicate effectively with residents, their families, and our team members to ensure seamless transitions and continuity of care.
- Maintain accurate and up-to-date records of resident care, including medications, treatments, and progress notes.
- Participate in quality improvement initiatives and contribute to the development of policies and procedures to enhance resident care and services.
Qualifications:
- Education and Experience: Graduate of an accredited nursing program and a current state license as a Licensed Practical Nurse (LPN) or Licensed Vocational Nurse (LVN).
- Skills and Abilities: Excellent communication and interpersonal skills, ability to work effectively in a fast-paced environment, and strong problem-solving and critical thinking skills.
- Preferred Qualifications: Experience in long-term care, gerontology, or a related field.
