Job Summary

Inspire health and serve with compassion as a Weekday Home Health Admission Nurse at Prisma Health. We are seeking a dedicated and compassionate Registered Nurse to join our team.

Key Responsibilities
  • Promote and maintain effective communication with other disciplines regarding patient care and agency function.
  • Document identified problems, interventions, discharge planning, physician orders, and care plan coordination accurately and efficiently.
  • Initiate and perform assessments and interventions at appropriate intervals, observing progress and adjusting interventions as needed.
  • Develop and implement discharge plans to address ongoing patient needs prior to discharge, maintaining patient confidentiality and adhering to agency policies.
  • Submit all paperwork and documentation according to agency policy, participating in chart audits and quality improvement projects.
Requirements
  • Associate degree in Nursing
  • 1 year of experience as a Registered Nurse
  • Registered Nurse license in South Carolina
  • Valid driver's license, acceptable three-year motor vehicle record, and certificate of auto insurance
  • BLS
Work Environment

This is a weekday home health admission nurse position, requiring reliable transportation and adherence to Prisma Health's driver safety policy and department driving policies.
