Home Health Physical Therapist Assistant

All Medical Personnel is seeking a skilled Physical Therapist Assistant to join our team in Oklahoma. As a key member of our home health care team, you will provide high-quality physical therapy services to patients in the comfort of their own homes.
Job Description:Role and Responsibilities:
- Collaborate with the interdisciplinary team to develop holistic plans of care focused on achieving patient physical therapy goals.
- Instruct, coach, guide, and intervene to support patients and families in achieving optimal outcomes.
- Promote optimal health, well-being, and quality of life of home health patients and their families through skilled intermittent physical therapy.
- Practice independently in patient homes using evidence-based practice for assessment, clinical decision-making, and coordination of care across the continuum.
- Act as case manager and team lead for patients requiring home health therapy services.
- Assess patients and collaborate with the interdisciplinary team to develop patient-centered plans of care.
- Provide physical therapy treatment to patients and families as ordered.
- Complete accurate clinical documentation to support services provided within specified timeframes.
- Provide physical therapy to pediatric, adolescent, adult, and geriatric patients based on principles of growth and development and life stages.
- Maintain communication regarding patients with the case manager and other disciplines via care conferences or other appropriate methods.
- Report acute medical problems and condition changes to the primary care provider.
- Provide supervision to Physical Therapy Assistants, Home Health Aides, and students in the clinical setting.
Requirements:
- 1-2 years of experience in an outpatient rehabilitation setting.
- At least two (2) years of physical therapy experience highly preferred.
- One (1) year experience in home health or other health setting with geriatric emphasis.
- Current license in Physical Therapy or ability to obtain prior to hire.
- American Heart Association Healthcare Provider BLS preferred upon hire, and required within 90 days of hire.
- Current Driver's license.
- Excellent customer service skills.
- Ability to work without direct supervision.
- Knowledge of physical therapy assessment and treatment methods.
- Knowledge of physical therapy modalities and equipment.
- Ability to maintain effective relationships with other healthcare professionals.
Estimated Salary: $65,000 - $80,000 per annum.
