Job Summary

We are seeking a highly skilled Cardiac Exercise Specialist to join our team at Health First. The successful candidate will be responsible for overseeing prescribed cardiac rehabilitation treatment, promoting optimal outcomes, and demonstrating an understanding of quality-driven, patient-centered care.

Key Responsibilities
  • Participate in the orientation and initial assessment of new cardiac rehabilitation patients, contributing to the development of the Individualized Treatment Plan with team members.
  • Assess patients' risk factors and provide support to aid patients in successful therapeutic lifestyle changes.
  • Document initial assessment findings, treatment session results, and reassessment outcomes, addressing clinical and behavioral data in the medical record.
  • Ensure all components of the Individualized Treatment Plan are completed and certified by medical director or referring physician every 30 days for the duration of the program.
  • Respond promptly to new or changing patient conditions and report to supervising and/or referring physician.
  • Review the plan of care and goals with patients and family/friends at each treatment session to emphasize the importance of inclusion and value of input from patients and family/friends.
  • Complete Focus on Therapeutic Outcomes at timely intervals to assess overall satisfaction with cardiac rehabilitation.
  • Manage the schedule for smooth throughput of cardiac rehabilitation assessments, re-assessments, and treatment sessions.
  • Review individualized treatment plans weekly to address progress and/or modifications.
Requirements

The ideal candidate will possess excellent communication and interpersonal skills, with the ability to work effectively in a multi-disciplinary team environment. A strong understanding of cardiac rehabilitation principles and practices is essential, as is the ability to adapt to changing patient needs and priorities.
