Job Title: Nurse Home Visitor

Public Health Solutions is seeking a skilled Nurse Home Visitor to join our team in Queens. As a Nurse Home Visitor, you will provide essential services to high-risk, low-income families, promoting healthy outcomes and empowering them to reach their full potential.

Job Summary:

The Nurse Home Visitor will assess the medical and social needs of pregnant women and infants, developing a personalized plan of care to address their unique challenges. You will work collaboratively with families, healthcare providers, and community organizations to ensure comprehensive support and resources.

Key Responsibilities:
  • Provide home visits to pregnant/parenting women and their families, offering education, case management, and support services.
  • Develop therapeutic relationships, utilizing reflection and motivational interviewing techniques to empower women and their families.
  • Assess physical, emotional, social, and environmental needs, identifying areas for improvement and developing targeted goals.
  • Collaborate with healthcare professionals, community organizations, and other stakeholders to ensure seamless service delivery.
  • Document all interactions, assessments, and interventions, ensuring accurate and timely Medicaid billing.
Requirements:
  • BSN required or associate's degree in nursing if currently enrolled in a BSN program.
  • Current New York State RN license (in good standing) required.
  • Current CPR certification required.
  • Bilingual English/Spanish preferred.
  • Two years of recent experience in maternal/child health, public health, home visiting, or mental/behavioral nursing preferred.
  • Home visiting experience preferred.
  • Excellent written and verbal communication skills.
  • Basic computer skills.
  • Valid driver's license and insured automobile may be required.
