Job Summary:

The City of New York is seeking a highly skilled and compassionate Nurse Home Visitor to join the Bureau of Maternal Infant and Reproductive Health. As a key member of our team, you will provide essential health services to women and their families, promoting healthy outcomes and empowering them to reach their full potential.

Responsibilities:

  • Assess the physical, emotional, and social needs of women and their families, utilizing the nursing process to develop and implement individualized care plans.
  • Provide counseling and education on prenatal and postpartum care, nutrition, parenting, childcare, family planning, and special health problems.
  • Link women and families with community resources and services, promoting self-sufficiency and healthy lifestyles.
  • Collaborate with other healthcare professionals to ensure comprehensive care and support.
  • Document patient interactions and progress, maintaining accurate and timely records.
  • Participate in ongoing learning and professional development, staying up-to-date on best practices and research in maternal and child health.

Requirements:

  • A license and current registration to practice as a Registered Professional Nurse in New York State.
  • A Bachelor of Science degree in Nursing from a regionally-accredited college or university.
  • A minimum of two years of full-time work experience in maternal/child health, pediatric health, or a closely related field.
