POSITION SUMMARY:

The Community Support Coordinator II plays a vital role in delivering comprehensive case management, facilitating referrals, and ensuring seamless collaboration with community and supportive service initiatives that foster economic independence for families, improve the quality of life for seniors and individuals with disabilities, and expand opportunities for residents. This position requires working alongside a multidisciplinary team to identify effective strategies and support families in various neighborhoods under the auspices of the U.S. Department of Housing and Urban Development.

KEY RESPONSIBILITIES:

  • Conduct initial assessments, enroll residents in the family support initiative, and gather information from various locations.
  • Collaborate with families to develop personalized plans, assist in accessing social services, and advocate for service delivery for a caseload of up to 100 individuals.
  • Maintain confidentiality, track resident information in Urban Strategies, Inc.'s data management system, and participate in community and program-related meetings.
  • Availability for occasional evening and weekend engagements, travel up to 25%, and perform administrative duties as needed.
  • Provide clients with resources and knowledge to access financial capital, including loans for achieving financial objectives such as housing and consumer products, aimed at building credit, reducing debt, and fostering wealth accumulation through entrepreneurship.
  • Offer advisory services in financial management and goal-setting.
  • Organize both in-person and virtual workshops for clients seeking resources in financial literacy and capital access.
  • Support and enhance the development of the Urban Strategies, Inc. Community Development Financial Institution (CDFI) client base.

REQUIRED QUALIFICATIONS:

  • Bachelor's degree in Social Work, Sociology, Psychology, or a related discipline.
  • Minimum of 3 years of experience in case management or direct service with low-income households; experience in health services is advantageous.
  • Valid driver's license and access to a vehicle for work-related purposes.
  • Commitment to strengths-based, family-centered service planning and an understanding of program philosophies.
  • Ability to manage confidential information with professionalism.
  • Independent and proactive work ethic, complemented by strong collaborative skills.
  • Flexibility to attend evening and weekend functions as required.
  • Strong verbal and written communication skills suitable for diverse communities.
  • Effective management of real-time priorities.
  • Excellent organizational, planning, and time management capabilities.
  • Ability to thrive in fast-paced, culturally diverse environments.
  • Proficiency in MS Office Suite (Advanced Excel) and other common software applications.
