We are seeking a highly skilled Primary Health Practitioner to join our team at Provider Healthcare in Missoula, Montana. As a Medical Care Specialist, you will play a vital role in delivering high-quality patient care and promoting overall well-being in our community.

**Salary Range:** $120,000 - $180,000 per year, negotiable based on experience.

**Job Summary:**

In this key role, you will be responsible for assessing patient needs, developing personalized treatment plans, and providing comprehensive care services. You will work collaboratively with healthcare professionals to ensure seamless care coordination and effective communication.

**Responsibilities:**

• Conduct thorough health assessments and create customized treatment plans for patients of all ages.
• Deliver preventative care services and follow-up visits to promote optimal patient outcomes.
• Work closely with healthcare teams to ensure timely and effective care coordination.

**Qualifications:**

• MT medical license as NP.
• MT DEA certificate.
• Current NP board certification by ANCC/AANP.
• CPR certification.
• Medical Spanish fluency (desired).
• 3+ years of experience as NP in primary care, family medicine, internal medicine, gerontology, or palliative care.
