About the Role:

Sunrise Senior Living is seeking a skilled and compassionate Wellness Nurse to join our team. As a Wellness Nurse, you will play a vital role in promoting the overall well-being of our residents, ensuring they receive the highest quality care and support.

Responsibilities:

  • Conduct regular wellness visits for all residents, monitoring their health and providing personalized care and support.
  • Collaborate with the Resident Care Director to develop and implement care plans that meet the unique needs of each resident.
  • Assist in maintaining accurate and up-to-date resident records, including medical histories and treatment plans.
  • Communicate effectively with residents, families, and healthcare professionals to ensure seamless care coordination.
  • Participate in quality improvement initiatives, identifying areas for improvement and contributing to the development of strategies to enhance resident care.

Requirements:

  • Graduate of an approved college or school of nursing.
  • Current state license as a professional Licensed Practical Nurse/Licensed Vocational Nurse (LPN/LVN).
  • Minimum of one year experience in a healthcare setting, preferably in a long-term care environment.
  • Excellent communication and interpersonal skills, with the ability to work effectively with residents, families, and healthcare professionals.
  • Ability to maintain confidentiality and handle sensitive information with discretion.
