As a seasoned therapist, you will provide education to patients, their families, and caregivers, while collaborating with other healthcare professionals to resolve issues and achieve specific patient goals.
As part of the interprofessional care team, the Physical Therapist II delivers direct patient care, ensuring adherence to safety protocols and guidelines to optimize patient safety, service quality, and overall outcomes.
This position also includes mentoring and guiding less experienced staff, including other therapists and technicians, and may serve as a clinical resource for students and clinical fellows.
The therapist will assist in gathering quality metrics and analytics, contributing to the development of best practices and process improvement initiatives as required.
Essential Functions:
Team Collaboration:
Contributes to a collaborative work environment by consistently offering assistance and responding positively to requests for help. Promotes healthy relationships within the team.
Provides guidance to the healthcare team when appropriate and applies feedback to enhance patient outcomes.
Utilizes therapeutic communication techniques, including active listening and teaching, to build rapport with patients, families, and the healthcare team, facilitating collaboration on care plans.
Partners with team members by effectively communicating and reporting relevant patient care information comprehensively.
Educates nursing and support staff, orients new team members, and assists in precepting students during clinical rotations.
Service Delivery:
Conducts thorough evaluations of patients across various diagnoses to determine their functional levels and develop appropriate treatment plans.
Implements the physical therapy treatment plan based on evaluation findings, providing services to complex patient populations.
Practices in alignment with principles of cultural diversity and inclusion, ensuring education is tailored to the learning needs of patients and their families.
Collaborates with the healthcare team to modify treatment plans and discharge planning to meet specific patient goals.
Advocates for patients and families to address clinical and ethical concerns using appropriate resources.
Balances caseloads among team members to ensure optimal patient care and outcomes.
Quality and Safety:
Regularly assesses patients, monitors progress towards treatment goals, and adjusts plans as necessary to address complications or limitations.
Provides oversight of physical therapy-related activities and adjusts plans to enhance quality outcomes while maintaining patient safety.
Identifies opportunities for improvement and contributes to meeting departmental and hospital targets for quality and safety.
Documentation and Compliance:
Maintains accurate and timely documentation of patient education and procedures in the patient record, ensuring compliance with all regulatory requirements.
Efficiently utilizes resources to maximize time and equipment usage, managing time effectively to meet productivity standards.
Professional Development:
Engages in innovative practices through evidence-based projects and actively participates in departmental initiatives and community service opportunities.
Identifies personal development needs and seeks opportunities for continuous learning and growth as a Physical Therapist.
Qualifications:
Graduate of an accredited education program recognized by the relevant credentialing body. A minimum of two years of experience as a Physical Therapist or completion of a comparable residency program is required.
Licenses and Certifications:
Current BLS certification and Texas State Licensure as a Physical Therapist are mandatory. A compact license is acceptable according to current regulations.
Skills and Abilities:
Demonstrates the necessary skills and competencies to perform assigned duties safely and effectively. Proficient in English communication, both written and verbal, with strong assessment and organizational skills.
Exhibits excellent interpersonal skills and the ability to work independently while managing complex patient care situations.
