Job Summary

We are seeking a dedicated and skilled Licensed Practical Nurse (LPN) to join our team on the night shift at The Osborn. The LPN will be responsible for delivering direct nursing care and administering medications and treatments within their scope of practice, under the direction of a Registered Nurse (RN).

Key Responsibilities
  • Provide direct nursing care to residents, ensuring their comfort and well-being.
  • Administer medications and treatments as prescribed by healthcare providers.
  • Monitor resident health status and report any changes or concerns to the RN or appropriate personnel.
  • Assist with the development and implementation of individualized care plans.
  • Collaborate with interdisciplinary team members to coordinate resident care.
  • Document all nursing interventions and observations accurately and promptly.
  • Adhere to infection control protocols and safety standards.
  • Serve as an advocate for residents and promote their rights and dignity.
  • Participate in staff meetings, training sessions, and quality improvement initiatives as required.
Qualifications
  • Valid LPN license in NYS and current CPR certification.
  • Proficiency in medication administration and nursing procedures.
  • Strong clinical assessment and critical thinking skills.
  • Excellent communication and interpersonal abilities.
  • Ability to work effectively as part of a multidisciplinary team.
  • Commitment to upholding ethical and professional standards of nursing practice.
  • Knowledge of regulatory requirements and best practices in long-term care.
