Key Responsibilities

The Licensed Practical Nurse (LPN) in Hospice is accountable for delivering high-quality patient care and executing technical nursing tasks in the patient's residence.
  • Administers skilled nursing care under the guidance of a registered nurse (RN) on a daily basis.
  • Dispenses prescribed medications and treatments in accordance with state regulations as required.
  • Assists the physician and/or RN in executing specialized procedures as necessary.
  • Supports patients with daily living activities and promotes self-care practices.
  • Communicates with the patient's physician and relevant personnel regarding significant changes in the patient's health status.
  • Conducts complex wound care if training has been completed for specific procedures as needed.
  • Receives and implements physician orders within the scope of practice for an LPN, adhering to hospice care standards without modifying the care plan.

Qualifications

Education: Associate Degree or equivalent certification

Licensure and Certification: Required

Licensure Requirements

  • Unrestricted licensure in the state of practice.
  • Current CPR certification is mandatory.
  • Valid driver's license and vehicle insurance, along with access to reliable transportation or public transit.

State-Specific Requirements

  • LA: Minimum of two years of full-time experience as an LPN.
