Job Title: Advanced Heart Failure Nurse Clinician
Job Summary: We are seeking an experienced nurse clinician to join our Advanced Heart Failure Program. The successful candidate will have a strong background in cardiovascular nursing and be able to provide comprehensive care to patients with advanced heart failure.
About the Role: As an Advanced Heart Failure Nurse Clinician, you will be responsible for conducting thorough patient assessments, managing symptoms, administering treatments, and educating patients and their families on medication management and lifestyle modifications. You will also collaborate with multidisciplinary teams to develop and implement individualized care plans tailored to each patient's unique needs.
Responsibilities:
- Collaborate with nursing, medical staff, and other health professionals in the development and review of standards and in planning, implementing, and evaluating patient care
- Develop and implement individualized care plans in collaboration with the multidisciplinary team
- Monitor and manage symptoms of advanced heart failure, including dyspnea, edema, fatigue, and chest pain
- Administer medications, including intravenous therapies, and provide patient education on medication management, potential side effects, and adherence
Requirements:
- Baccalaureate degree in nursing required
- Minimum 6 months experience in direct nursing care or management/supervision of cardiovascular medical/surgical patients
- Excellent written and verbal communication skills
- Experience with electronic health records preferred
Salary: Estimated annual salary range $85,000 - $110,000 per year, based on experience and qualifications.
