About the Role
Nuvance Health is seeking a skilled Occupational Therapist to join our Home Healthcare team. As an Occupational Therapist, you will play a vital role in helping patients recover from illness or injury and regain their independence in the comfort of their own homes.
Key Responsibilities
- Develop and implement personalized plans of care in partnership with patients, caregivers, and physicians.
- Provide occupational therapy services as ordered by physicians and communicate with healthcare practitioners as needed.
- Maintain accurate records of patient visits, including evaluations, plans, and client responses to therapy.
- Participate in case management conferences to coordinate occupational therapy with the overall plan of care.
- Provide education and counseling to patients and family members on occupational therapy treatment regimens.
- Identify patient and family needs for other home health services and make referrals as necessary.
- Participate in quality assurance and performance improvement programs, as well as agency-sponsored in-service training.
- Supervise care provided by certified occupational therapy assistants and home health aides as needed.
- Re-evaluate patient progress and treatment regimens, making changes as indicated and under physician recommendations.
- Fulfill all compliance responsibilities related to the position, including in-service training.
Requirements
- Minimum one year of experience in occupational therapy.
- Desirable: Experience in a qualified home health agency.
Working Conditions
- Some exposure to dirt, odors, noise, human waste, etc.
- Significant manual skills and finger dexterity.
- Some occupational risk.
- Extremely heavy effort, exerting over 125 pounds of force.
