Job Title: Home Health Speech Language Pathologist

We are seeking a skilled Home Health Speech Language Pathologist to join our team at Alternate Solutions Health Network LLC. As a Home Health Speech Language Pathologist, you will provide high-quality speech therapy services to patients in their homes, helping them to recover from speech and language disorders.

Job Summary:

The Home Health Speech Language Pathologist will work closely with patients, families, and healthcare teams to develop and implement individualized speech therapy plans. You will assess patients' communication needs, create treatment plans, and provide therapy sessions to help patients achieve their goals.

Responsibilities:
  • Assess patients' communication needs and develop individualized speech therapy plans
  • Provide speech therapy sessions to patients in their homes
  • Work closely with patients, families, and healthcare teams to ensure effective communication and coordination of care
  • Document patient progress and communicate with healthcare teams as needed
  • Participate in interdisciplinary team meetings to discuss patient care and treatment plans
Requirements:
  • Master's degree in Speech-Language Pathology or related field
  • Current licensure in the state of employment
  • Minimum one year of experience as a Speech Language Pathologist in a home health setting
  • Valid driver's license and auto insurance
