Job Summary:
The Nurse Specialist - Renal Care Coordination will be responsible for providing clinically competent care to patients with kidney disease, including assessment, planning, intervention, and evaluation. This role will also involve collaborating with interdisciplinary teams to develop and implement effective care plans, as well as educating patients and their families on renal care and treatment options.

Responsibilities:
* Provide direct patient care, including administering medications, monitoring vital signs, and conducting assessments
* Develop and implement individualized care plans, taking into account patient preferences and needs
* Collaborate with healthcare teams to ensure seamless transitions of care and coordination of services
* Educate patients and their families on renal care and treatment options, as well as provide support and guidance throughout the treatment process
* Participate in quality improvement initiatives and contribute to the development of policies and procedures related to renal care

Requirements:
* RN licensure in the state of Georgia
* 6 months experience in acute dialysis as a RN
* Graduate of an accredited School of Nursing
* Excellent communication and interpersonal skills, with ability to work effectively in a fast-paced environment
* Strong critical thinking and problem-solving skills, with ability to prioritize tasks and manage multiple responsibilities
* Ability to lift up to 30 lbs and move equipment as needed

Work Environment:
