- Collaborate with the healthcare team to develop and implement individualized care plans, ensuring that residents receive comprehensive and coordinated care.
- Communicate effectively with residents, families, and healthcare professionals to ensure seamless care transitions and address any concerns or questions.
- Participate in ongoing education and training to stay up-to-date on best practices and industry developments.
- Graduate of an accredited School of Nursing and currently licensed as a Nurse in the State.
- Previous work experience in a long-term care setting, with supervisory experience and organizational skills preferred.
- Excellent technical assessment and documentation skills, as well as leadership qualities and good communication skills.
- Ability to communicate in English, both verbally and in writing, and possess sufficient communication skills to perform the tasks required.
