Primary Care Physician

Key Responsibilities:- Provide high-quality, patient-centered care to a panel of up to 450 patients
- Develop and implement individualized care plans to improve patient outcomes
- Collaborate with care teams to ensure seamless care coordination and communication
- Participate in quality improvement initiatives to enhance patient care and safety
- Stay up-to-date with the latest clinical guidelines and evidence-based medicine
- MD or DO in Internal Medicine, Family Medicine, Geriatrics, or similar specialty
- Current, active MD licensure in the state of employment
- A minimum of 1 year of clinical experience in geriatric, adult, or family practice setting
- Board certification in Internal Medicine, Family Medicine, Geriatrics, or similar specialty
- Basic Life Support (BLS) certification
- Current DEA number for schedule II-V controlled substances
