About the Role

We are seeking a highly skilled Registered Nurse to join our team as a Case Manager in Home Health Care. As a key member of our team, you will be responsible for coordinating and directing the care of our patients in their homes, ensuring they receive the highest level of care and support.

Key Responsibilities
  • Conduct comprehensive assessments of patients' physical, functional, and psychosocial needs
  • Develop and implement individualized care plans in collaboration with patients, families, and healthcare providers
  • Provide education and support to patients and families to promote independence and self-care
  • Collaborate with interdisciplinary teams to ensure seamless care coordination and communication
  • Monitor and evaluate patient outcomes, making adjustments to care plans as needed
Requirements
  • Current Registered Nursing License in the State of Practice
  • Minimum of one year of relevant clinical RN experience
  • Strong computer and technology skills
  • Ability to work independently and as part of a team
  • Excellent communication and interpersonal skills
Preferred Qualifications
  • Home Health Case-Manager Certification
  • Recent, relevant experience in a Medicare-certified home health agency as a case-manager
  • Bachelor's degree in nursing
