We are seeking a skilled RN Care Advisor to join our team at Chapters Health System. As a key member of our care team, you will provide compassionate and high-quality care to patients and their families in their homes, assisted living facilities, nursing homes, or hospitals.
Key Responsibilities- Make after-hours visits to patients to assess their needs and provide direct care.
- Collaborate with other healthcare professionals to promote quality and continuity of patient care.
- Provide education to patients, families, and caregivers about disease processes, palliative interventions, and safety practices.
- Document patient information and care provided according to company policy and procedure.
- Current license as RN in the state where the employee will be working.
- Minimum of one (1) year nursing experience; hospice or hospital experience preferred.
- Ability to effectively communicate in English (verbal and written); bi-lingual in Spanish preferred.
- Mobile Driver - Valid driver's license and automobile insurance per Company policy.
- Reliable transportation to meet patients' needs.
