Position Overview

The Healthcare Outreach Coordinator is essential to our organization, collaborating closely with Physicians and medical personnel to conduct Annual Wellness Visits for our Medicare demographic. This role involves executing guided procedures under the oversight of local healthcare teams to address critical components of the Annual Wellness Visit, identifying care gaps, safety issues, and social requirements. The Coordinator is responsible for preparing patient records and relaying any patient concerns to the Providers. Additionally, this position may involve outreach efforts to eligible patients who have yet to complete their Annual Wellness Visit.

Available Locations: Various locations

Incentive: $500 Sign-On Bonus after 90 days

Key Responsibilities

  • Ensure patient confidentiality in compliance with HIPAA regulations.
  • Participate in ongoing training to remain informed about best practices for conducting Annual Wellness Visits and recognizing patient needs.
  • Work alongside the healthcare team to create tailored care plans based on findings from the Annual Wellness Visit.
  • Utilize electronic health record (EHR) systems for accurate and efficient documentation of patient information.
  • Exhibit empathy and active listening skills to foster trust and rapport with patients.
  • Provide patient education on preventive care, healthy lifestyle choices, and disease management.
  • Engage in quality improvement initiatives aimed at enhancing patient experiences and optimizing clinical outcomes.
  • Maintain a professional demeanor while thriving in a fast-paced, team-oriented environment.
  • Perform additional duties as assigned.

Qualifications

  • High School Diploma required.
  • Healthcare experience is preferred but not mandatory.
  • Strong organizational skills are essential.
  • Excellent communication and interpersonal skills are necessary.
  • A friendly and patient-oriented personality is crucial.
  • Comfortable with phone communication.
  • Proficient with computer applications and typing.
  • Ability to excel in a high-paced environment.
  • Complete OSHA Bloodborne Pathogens training upon hire and annually thereafter.
  • Adhere to OSHA standards for personal protective equipment (PPE) and proper hand hygiene protocols.

Physical & Mental Requirements:

  • Ability to sit for extended periods.
  • Ability to stand for extended periods.
  • Ability to use fine motor skills for operating office equipment and/or machinery.
  • Ability to comprehend instructions both verbally and in writing.
  • Ability to apply logical reasoning for problem-solving.
  • Willingness to travel to various locations as required to support business needs.
