Job Title: Experienced APP

We are seeking an experienced APP (APRN or PA) to oversee the West Volusia County Transitional Care Clinics. As a key member of our team, you will be responsible for managing complex, high-risk patients and collaborating with a multidisciplinary team to reduce inpatient readmissions.

About the Role:
  • Manage a caseload of high-risk patients with a focus on reducing inpatient readmissions
  • Collaborate with a multidisciplinary team, including pharmacists, respiratory therapists, social workers, and diabetic educators
  • Develop and implement care plans to improve patient outcomes
  • Provide education and support to patients and their families

Requirements:
  • 3+ years of experience as an APP (APRN or PA)
  • Board Certification: Physician Assistant or Nurse Practitioner
  • Active FL medical license (or ability to secure active, unrestricted licensure)
  • DEA License required (or ability to obtain)
  • Strong communication skills
