Job Summary

We are seeking an experienced APP (APRN or PA) to lead our clinical team in the West Volusia County Transitional Care Clinics. As a key member of our team, you will be responsible for overseeing the delivery of high-quality patient care, managing complex patient cases, and collaborating with our interdisciplinary team to achieve exceptional patient outcomes.

Key Responsibilities
  • Lead the clinical team in the West Volusia County Transitional Care Clinics, providing guidance and support to ensure seamless patient care.
  • Manage complex patient cases, developing and implementing individualized care plans to achieve optimal patient outcomes.
  • Collaborate with our interdisciplinary team, including pharmacists, respiratory therapists, social workers, and diabetic educators, to ensure comprehensive patient care.
  • Develop and maintain strong relationships with patients, families, and healthcare providers to ensure continuity of care.
  • Participate in quality improvement initiatives, contributing to the development and implementation of evidence-based practices to enhance patient care.
Requirements
  • 3+ years of experience as an APP (APRN or PA)
  • Board Certification: Physician Assistant or Nurse Practitioner
  • Active FL medical license (or ability to secure active, unrestricted licensure)
  • DEA License required (or ability to obtain)
  • Strong communication and leadership skills
