Qualifications:
1. Must be a graduate from an accredited institution with a Master's degree in Communication Disorders recognized by ASHA and the state board.
2. Must possess a valid license and/or registration in the applicable state.
Key Responsibilities:
1. Conduct thorough speech therapy assessments as per physician directives.
2. Accurately document findings using standardized evaluation formats, adhering to all guidelines.
3. Develop a detailed treatment plan that includes long-term and short-term objectives, frequency, duration, and therapeutic methods.
4. Ensure physician orders are secured for evaluations, treatments, recertifications, and discharges.
5. Deliver comprehensive therapy to patients, utilizing appropriate modalities and adjusting treatment as necessary while following safety precautions.
6. Record patient progress in the approved format promptly and accurately.
7. Update treatment plans monthly in a standardized format in a timely manner.
8. Document patient discharges and any necessary in-services.
9. Complete all required documentation as mandated by the facility.
10. Maintain care plans and necessary information for the MDS as required by the facility.
11. Attend facility and rehabilitation meetings as directed by supervisors or regional management.
12. Participate in facility committee meetings as required.
13. Act as a rehabilitation representative within the interdisciplinary team.
14. Assist in scheduling patient appointments.
15. Conduct screenings as necessary.
16. Ensure timely and ethical billing for all speech therapy services provided.
17. Cover the caseload of other therapists during their absence as needed.
18. Supervise junior staff members or students in compliance with state and federal regulations, co-signing notes as required.
19. Foster professional relationships with colleagues, facility staff, patients, and their families.
20. Provide educational in-services regarding patient care or speech therapy services to rehabilitation staff, facility personnel, and the community as needed.
21. Adhere to all facility policies, procedures, and safety protocols.
22. Maintain patient confidentiality and comply with Federal Resident Rights and HIPAA regulations.
23. Arrive at work punctually, adhere to scheduled hours, and maintain a professional demeanor at all times.
24. Meet productivity standards, complete productivity reports, and submit them promptly.
25. Follow all facility Human Resource policies regarding breaks, overtime, sick leave, and time-off notifications.
26. Promote a positive work environment by communicating professionally to foster good relationships with customers, clients, colleagues, and management.
27. Perform any additional duties assigned by the department supervisor, lead therapist, or regional manager.
28. Comply with all other facility written and verbal systems and operations.
