Job Summary:

The Transplant Coordinator is a vital member of the healthcare team, responsible for coordinating the clinical aspects of transplant patient and living donor care. This role requires collaboration with the healthcare team to develop patient care plans for individuals undergoing solid organ transplantation. The Transplant Coordinator ensures the coordination of transplant patient and living donor care, acts as a liaison between the transplant program and dialysis facilities, and promotes health and disease prevention. This role also involves mentoring, delegating, and providing feedback to the support team.

Responsibilities:
  • Collaborate with the healthcare team to develop patient care plans for transplant patients.
  • Coordinate the clinical aspects of transplant patient and living donor care.
  • Act as a liaison between the transplant program and dialysis facilities.
  • Promote health and disease prevention.
  • Mentor, delegate, and provide feedback to the support team.
Requirements:
  • Bachelor's Degree in Nursing.
  • License/Registration issued by Florida or Compact State.
  • Registered Nurse certification.
  • Two years of critical care experience.
  • Experience in nursing and transplantation.
Preferred Skills:
  • Expertise in transplantation and end-stage organ disease management.
  • Evidenced-based practice related to current research, guidelines, and evidence related to transplant and organ disease management.
  • Diagnostic skills to proficiently initiate and interpret diagnostic tests relevant to transplant and organ disease.
  • Electronic Medical Record (EMR) documentation.
  • Postoperative Monitoring.
