About the Role

As a Field HCBS Wait List Care Coordinator, you'll play a critical role in ensuring that individuals receive the necessary services in a supportive, effective, efficient, timely, and cost-effective manner.

Key Responsibilities
  • Establish person-centered, goal-oriented, and culturally relevant care plans to address the unique needs of each individual.
  • Coordinate case management, disease management, discharge planning, transition planning, and address social determinants of health to ensure seamless care transitions.
  • Work closely with community service coordinators, healthcare teams, and other stakeholders to ensure quality, coordinated care.
  • Advocate for individuals and families to ensure their needs and choices are fully represented and supported by the healthcare team.
  • Conduct home visits and in-person visits to assess and implement care strategies, as needed.

Requirements
  • Bachelor's degree in social work, rehabilitation, nursing, psychology, special education, gerontology, or a related human services field.
  • 1+ years of experience working with individuals with long-term care needs.
  • 1+ years of experience working in a community health setting in a healthcare role.
  • Intermediate level of computer proficiency, including Microsoft Outlook, Teams, and multiple web applications.
  • Ability to travel 75% of the time for this role.
Preferred Qualifications
  • Licensed Social Worker or clinical degree.
  • 1+ years of experience working with individuals receiving services on one of the home and community-based waivers in KS.
  • Background in managing populations with complex medical or behavioral needs.
  • Experience with electronic charting and arranging community resources.

  • Strong communication and customer service skills, both in person and via phone.
  • Ability to work independently and maintain good judgment and accountability.
  • Demonstrated ability to work well with others.
  • Strong organizational and time management skills.
  • Ability to multi-task and prioritize tasks to meet all deadlines.
