About the Job
ECHN is seeking a skilled and compassionate Registered Nurse to join our team as a Home Health Nurse. As a member of our team, you will have the opportunity to make a meaningful difference in the lives of patients and their families.
Job Summary
The successful candidate will be responsible for providing high-quality patient care and education to patients in their homes. This will include assessing patient needs, developing and implementing individualized care plans, and coordinating with other healthcare professionals to ensure seamless care transitions.
Responsibilities:
- Conduct thorough assessments of patients to identify their unique needs and develop personalized care plans.
- Provide direct patient care and education to patients and their families, promoting independence and self-care.
- Collaborate with other healthcare professionals, including physicians, therapists, and social workers, to coordinate care and achieve optimal outcomes.
- Document patient progress and communicate effectively with patients, families, and healthcare teams.
- Maintain accurate records and adhere to regulatory requirements and agency policies.
Requirements
To be considered for this rewarding role, applicants must possess:
- A valid RN license in the state of Connecticut.
- A minimum of one year of recent medical/surgical nursing experience, with a focus on community health nursing.
- Excellent communication, interpersonal, and critical thinking skills.
- A strong commitment to providing patient-centered care and advocating for patients' rights and needs.
- The ability to work independently and as part of a multidisciplinary team.
Salary and Benefits
We offer a competitive salary range of $65,000 - $85,000 per year, depending on experience, plus excellent benefits, including comprehensive health insurance, paid time off, and opportunities for professional growth and development.
