Supportive Living Assistant

Position Summary:
This role is dedicated to delivering personal assistance and health-related services aimed at preserving the physical and emotional health of residents. The Supportive Living Assistant adheres to the personalized service plan for each resident and accurately records the services rendered.
Key Responsibilities:
- Provide assistance with treatments and daily living activities as directed by the Clinical Director according to the individualized care plan. This includes but is not limited to:
- Support with dressing, grooming, bathing, eating, toileting, mobility, and transferring.
- Assist with morning and evening care routines, including oral hygiene as necessary.
- Incontinence management.
- Support with assistive devices, eyeglasses, and hearing aids.
- Administer medications as instructed by the Registered Nurse.
- Assist with medications, treatments, and daily living activities as assigned by the Clinical Director/Manager following the individualized care plan.
- Monitor, document, and report any changes in the physical, cognitive, or emotional state of residents.
- Encourage independence among residents by offering choices and promoting self-help skills.
- Collaborate effectively with colleagues in a dynamic environment, providing support to all staff as directed by the Clinical Director or Registered Nurse.
- Train and mentor assigned staff members.
- Complete scheduled cleaning responsibilities.
- Lead and facilitate resident activities as required.
- Respond appropriately to safety hazards, call lights, requests, and reassignment of tasks.
- Perform other duties as assigned.
Minimum Qualifications:
- Must be 17 years of age or older.
- Effective verbal and written communication skills.
- Ability to adhere to all safety protocols, regulations, policies, and procedures of the facility, including but not limited to: Vulnerable Adult and Abuse Policy, HIPAA, Resident's Bill of Rights, Universal Precautions, AWAIR, OSHA, Right to Know, SDS, ADA, HUD Policy and Procedures, Fair Housing laws and regulations.
Preferred Qualifications:
- High school diploma, GED, or higher education.
- Prior experience working with senior populations.
- Licensure/Certification: Certified Nursing Assistant (CNA) and/or Home Health Aide.
