Position Overview: Transition Navigator

The Transition Navigator plays a crucial role in enhancing the lives of youth and young adults who have navigated the complexities of the foster care system. This position is dedicated to fostering self-sufficiency and supporting individuals aged 14 to 29 in achieving their personal and professional goals.

Key Responsibilities:

  • Establish rapport and trust with clients to empower them in their journey.
  • Conduct assessments to identify strengths and areas for improvement, collaborating with clients to create actionable Future Plans.
  • Provide hands-on coaching in real-world settings, facilitating skill development and decision-making.
  • Assist clients in managing and reducing high-risk behaviors, including substance use and self-harm.
  • Implement trauma-informed care practices, recognizing the impact of past experiences on current functioning.
  • Connect clients with community resources, ensuring they have access to necessary support services.
  • Maintain accurate documentation of client progress and outcomes related to transitional goals.

Qualifications:

  • A Master's degree in social work, psychology, or a related field, or a Bachelor's degree with relevant experience.
  • Experience working with youth and young adults in foster care is preferred.
  • Strong communication skills, both verbal and written, with the ability to engage effectively with diverse populations.
  • Proficient in navigating community resources and establishing connections for client support.
  • Ability to work in a dynamic environment and adapt to changing client needs.

Additional Requirements:

  • Valid driver's license and reliable transportation for client visits.
  • Commitment to maintaining professional standards and ethical guidelines.
  • Willingness to participate in ongoing training and professional development.
