About the Job

We are seeking a highly skilled Healthcare Specialist - Palliative Care to join our team at the Medical College of Wisconsin.

This is an excellent opportunity for individuals who are passionate about providing high-quality care to patients with serious illnesses. Our palliative care team works collaboratively with physicians, nurses, and other healthcare professionals to provide comprehensive care to patients and their families.

Job Summary

The Palliative Care Specialist will work in collaboration with our multidisciplinary team to deliver high-quality, patient-centered care. This includes conducting comprehensive assessments, developing treatment plans, and coordinating care with other healthcare providers.

Our ideal candidate will have a strong background in nursing or medicine, with experience in palliative care and/or hospice care. A master's degree in nursing or a related field is required.

Responsibilities
  • Conduct comprehensive assessments of patients' physical, emotional, and spiritual needs
  • Develop individualized treatment plans that address patients' goals and preferences
  • Collaborate with other healthcare providers to coordinate care and ensure seamless transitions
  • Provide education and support to patients and their families
Requirements

To be successful in this role, you will need:

  • A Master's degree in nursing or a related field
  • At least 5 years of experience in palliative care and/or hospice care
  • Current RN licensure in the state of Wisconsin
  • Certification in hospice and palliative nursing (CHPN) or physician assistant (PA-C)
