Care Management Role Overview

RHA Health Services is seeking a skilled Care Manager to join our team. As a Care Manager, you will be responsible for providing whole person, integrated care management in service sites, homes, and communities through face-to-face and other interactions.

Key Responsibilities:

  • Conduct comprehensive assessments and develop tailored care management plans with individuals and their support teams.
  • Monitor and facilitate ongoing care management plan implementation, ensuring high-quality services and positive outcomes.
  • Implement workflows, policies, and procedures that promote whole person care and maintain a positive, professional image in the work setting and community.
  • Cross-functionally collaborate with integrated care teams, community organizations, and RHA departments to ensure successful care management implementation.
  • Maintain a person-centered philosophy, focusing on individual strengths, needs, and preferences to promote health, wellness, and safety.

Requirements:

  • A master's degree in a human service field and at least one year of full-time experience working with the MH/SU/IDD population, or a bachelor's degree in a human service field and at least two years of full-time experience working with the MH/SU/IDD population, or a bachelor's degree in a non-human service field and at least four years of full-time experience working with the MH/SU/IDD population.
  • Two years of prior LTSS and/or HCBS coordination, care delivery monitoring, and care management experience for care managers serving members with LTSS needs.
