Job Summary

We are seeking a skilled Physical Therapy Assistant to join our team at Bradford Hills Nursing & Rehabilitation Center. As a Physical Therapy Assistant, you will play a vital role in supporting our Physical Therapist in delivering high-quality patient care.

Key Responsibilities
  • Treatment Support: Assist the Physical Therapist in implementing treatment plans, including exercises, modalities, and mobility training, to help patients achieve their rehabilitation goals.
  • Monitoring: Observe patients' progress and report any changes or concerns to the supervising Physical Therapist, ensuring timely interventions and adjustments to treatment plans.
  • Documentation: Maintain accurate and up-to-date documentation of patient progress, interventions, and evaluations, utilizing electronic medical records (EMRs) such as Net Health/PCC.
  • Education: Educate patients, family members, and care staff on safe mobility techniques and exercises, promoting continued progress and independence.
  • Team Collaboration: Collaborate with the interdisciplinary team, following the guidance of the Physical Therapist and in cooperation with other rehab staff, to ensure comprehensive patient care.
Requirements

To be successful in this role, you will need to possess excellent communication and interpersonal skills, as well as the ability to work effectively in a team environment. You should also be comfortable with electronic medical records and have a strong understanding of rehabilitation principles and practices.
