About the RoleThis is a full-time opportunity to work as a provider member within our interdisciplinary team, delivering high-quality home-based medical care to patients in need.
Key Responsibilities- Conduct comprehensive assessments and develop patient management care plans
- Collaborate with patients, caregivers, and healthcare professionals to ensure seamless care
- Prescribe diagnostics and interventions to prevent unnecessary hospitalizations
- Provide education and support to patients and staff
- Participate in quality improvement initiatives and HEDIS measure campaigns
- Completion of a Nurse Practitioner accredited program
- Current state RN license and NP license
- Access to reliable transportation
- 2+ years of clinical experience in primary care, home-based medical care, geriatrics, or palliative care
- Knowledge of long-term care, community resources, and cost-effective alternatives
