Comunilife Inc. is seeking a dedicated Case Manager to join its Life is Precious program in Queens. As a Case Manager, you will play a vital role in supporting Latina teens and their families in preventing suicide attempts and ideation.
Key Responsibilities:- Provide outreach and engagement with program participants to ensure continuity of mental health treatment and encourage participation in the Life is Precious program.
- Offer case management and counseling services to connect clients with necessary resources.
- Develop and implement age-appropriate programming to promote healing, cultural pride, and awareness among participants.
- Collaborate with community agencies, public officials, and organizations to enhance program effectiveness.
- Represent the program to community providers and provide information and training relevant to the program focus and populations.
