We are seeking an experienced Advanced Practice Provider to join our team, providing advanced wound care to patients in their home setting.
Key Responsibilities- Assess health status for wound care patient populations using advanced practice knowledge and skills.
- Develop and implement an individualized patient plan of care, including coordination of care between visits.
- Conduct comprehensive patient assessments, paying special attention to factors that increase the risk of wound formation.
- Order necessary medications and provide clear instructions for their use.
- Educate nursing staff and maintain open communication with primary care physicians, families, and clinical teams as required.
- Contribute to effective and efficient practice management policies and practices that support patient access to medical care.
- Graduate of an accredited nurse practitioner or physician assistant program.
- A clinical background in adult, family, geriatric, podiatry, or vascular specialties.
- At least one year as an advanced practice provider.
- Board Certified/Prepared through AANP, ANCC or NCCPA.
- Wound care experience a plus.
- Nurse Practitioner national certification as ANP, FNP, or GNP required or ability to obtain.
- Current, unrestricted license in the state of assignment or the ability to obtain license.
- Ability to obtain and/or maintain State DEA licensure/Prescriptive Authority post-where applicable.
- Microsoft office and CRM experience preferred.
