About the Role

We are seeking a skilled Speech Language Pathologist to join our team at VNA Health Group. As a Home Health Speech Language Therapist, you will be responsible for providing high-quality speech and language therapy services to patients in their homes or alternate community settings.

Key Responsibilities
  • Conduct comprehensive assessments and evaluations of patients to identify speech and language therapy needs.
  • Develop and implement individualized plans of care to address patient needs and goals.
  • Provide speech and language therapy services to patients in their homes or alternate community settings.
  • Collaborate with healthcare teams to ensure seamless care coordination and communication.
  • Document patient progress and outcomes in a timely and accurate manner.
Requirements
  • Master's degree in Speech-Language Pathology from an accredited program.
  • Current licensure as a Speech-Language Pathologist in the state of New Jersey.
  • CCC-SLP certification.
  • 2-3 years of prior clinical experience and community-based experience preferred.
  • Strong computer skills and ability to navigate electronic medical records.
  • Excellent communication and interpersonal skills.
  • Valid NJ driver's license.
