About the Role

We are seeking a highly skilled and compassionate Licensed Therapist PRN to join our team at Perimeter Healthcare. As a key member of our behavioral health team, you will play a vital role in providing high-quality care to our patients.

Responsibilities
  • Conduct psychosocial assessments of new patients to develop personalized treatment plans
  • Maintain contact with family members and significant others to provide education, support, and advocacy
  • Coordinate with community-based social and mental health agencies to ensure seamless care transitions
  • Facilitate group therapy sessions and provide individualized support to patients
  • Develop and implement discharge plans to reduce re-hospitalization rates
Requirements
  • Master's degree in Social Work (MSW) or related field from an accredited institution
  • Unrestricted licensure in social work (LMSW or LPC) or equivalent
  • At least one year of experience in a hospital setting with psychiatric experience preferred
  • Successful completion of CPR and Handle with Care Training within 30 days of employment
