Key Responsibilities
  • Coordinate and complete Minimum Data Set (MDS) assessments to ensure accurate and comprehensive care planning.
  • Develop and implement resident-centered care plans that meet the unique needs and goals of each individual.
  • Collaborate with healthcare professionals to ensure seamless care transitions and effective communication.
  • Monitor and evaluate the effectiveness of care plans, making adjustments as needed to ensure optimal outcomes.
Requirements
  • Current licensure as a Registered Nurse (RN) in the state of employment.
  • 2+ years of experience as an RN, with a strong background in assessment and care planning.
  • Experience working in long-term care or a related field is preferred, but not required.
