Job Summary:

The Clinical Care Coordinator will work closely with healthcare professionals, patients, and families to coordinate care and services. This includes assessing patient needs, developing and implementing care plans, and monitoring patient progress. The ideal candidate will have excellent communication and interpersonal skills, with the ability to work effectively in a fast-paced environment.

About the Role:

The Clinical Care Coordinator will be responsible for:

  • Coordinating patient care and services, including assessments, care planning, and monitoring patient progress.
  • Developing and implementing care plans that meet the unique needs of each patient.
  • Working closely with healthcare professionals, patients, and families to ensure seamless coordination of care.
  • Providing education and support to patients and families to promote health and well-being.
  • Collaborating with other departments to ensure effective communication and coordination of care.

Requirements:

To be successful in this role, you will need:

  • A degree in a relevant field, such as nursing or healthcare administration.
  • Experience working in a healthcare setting, preferably in a clinical or social services role.
  • Excellent communication and interpersonal skills, with the ability to work effectively in a fast-paced environment.
  • Ability to develop and implement care plans that meet the unique needs of each patient.
  • Strong analytical and problem-solving skills, with the ability to prioritize tasks and manage multiple projects.
