We are seeking a skilled Licensed Practical Nurse to join our Home Care team at Hackensack Meridian Health. As a Home Care LPN, you will provide high-quality nursing care to patients in their homes, focusing on one patient at a time. This role offers a flexible schedule and a setting with fewer distractions, allowing you to do what you do best.

Responsibilities
  • Provide nursing care to patients in their homes through assessment, development, implementation, and evaluation of care plans.
  • Work under the direction of an RN to provide home nursing care needs of assigned patients.
  • Comply with physician orders in a timely manner and chart, audit, and demonstrate compliance on frequency and timely admission of referrals.
  • Assess patient acuity in a timely and accurate manner and communicate with healthcare team members through secure messaging.
  • Independently prioritize and complete routine assignments consistently, accurately, and within guidelines.
  • Complete patient care-related activities within scheduled frequencies and issue appropriate frequencies directed by physicians.
  • Assure assessment, planning, intervention, and evaluation of patient care are reflective of policy, procedures, and standard of care.
  • Teach patients and families based on identified health needs and chart, audit, and demonstrate documentation of teaching goals and plans.
  • Report patients' problems to primary nurse or nursing management personnel with possible solutions.
Qualifications
  • Graduate of an LPN program or equivalent.
  • Six (6) months of experience.
  • Home care experience preferred.
  • Licensed Practical Nurse.
  • AHA Basic Health Care Life Support HCP Certification.
