About the Job:

To Help Everyone Health & Wellness Centers is a nonprofit community clinic in South Los Angeles, dedicated to providing high-quality, low-cost healthcare and preventative education to all. We are currently seeking a Program Director RN to lead our Enhanced Care Management (ECM) program.

Key Responsibilities:

  • Manage and oversee the day-to-day operations of the ECM program, ensuring smooth implementation of policies and procedures.
  • Collaborate with cross-functional teams to develop and implement strategies that improve patient outcomes and reduce healthcare costs.
  • Maintain effective communication with stakeholders, including patients, families, and healthcare providers.
  • Analyze data to identify trends and areas for improvement, and develop targeted interventions to address these issues.

Requirements:

  • Experience working in a managed care environment, preferably with Medi-Cal underserved and culturally diverse populations.
  • Proven track record of successfully managing multiple projects simultaneously and prioritizing tasks effectively.
  • Strong analytical and problem-solving skills, with ability to think critically and outside the box.

Estimated Salary: $140,000 - $170,000 per year
