Job Title: Case Manager III

We are seeking a skilled Case Manager III to join our team. As a Case Manager III, you will work with clients to address mental health and substance use symptoms, including opioid usage, through mobile intervention and office-based work. You will also provide support to therapists by taking responsibility for level of care transfers to residential facilities across the state.

Key Responsibilities:

  • Accept referrals directly from ASAP and Sarpy County Law Enforcement.
  • Work with clients to assess their holistic health care and social service needs, strengths, abilities, and desires.
  • Develop care coordination plans with clients that are realistic, achievable, goal-oriented, and focused on client needs and desires.
  • Coordinate services, resources, and information with clients and on their behalf when needed.
  • Maintain all reports required of the position.
  • Interact with primary care providers, behavioral health providers, and other social service providers to make needed connections for services, provide a conduit for information sharing, and organize case conferences to review the care of mutual members.
  • Work within the team model, utilizing the expertise of team members when appropriate for each client.
  • May include teaching life skills or budgeting classes to clients.
  • Provide direct intake/assessment of clients' presenting problems and barriers to recovery.
  • Provide information and referral to address client needs.
  • Provide problem-solving and crisis intervention to clients.
  • Provide referral and coordination for issues that may interfere with therapy attendance, including making follow-up phone calls to clients who miss their appointments.
  • The ability to communicate positively with community members regarding coordination of services.
  • Display a comfort level in working with stressful individual/family dynamics.
  • Displays a courteous and caring attitude at all times to the clientele, volunteers, and visitors of the agency.
  • Cooperates and collaborates with program area staff, volunteers, and other Agency staff.
  • Is dependable and punctual regarding scheduling and attendance.
  • Abides by all specific program and Agency procedures, policies, and requirements.
  • Develops personal and program-related skills through participation in internal and external training opportunities.
  • Creates, maintains, and shares as appropriate a dynamic self-care plan.
  • Strives to make connections between the agency and the larger community whenever possible to contribute to the agency's ongoing fundraising and friend-raising efforts.

Requirements:

  • Bachelor's degree in counseling, social work, or a related field.
  • Valid driver's license, acceptable driving record.
  • Training in case management is preferred.

Work Schedule: Monday through Friday, 8:00 a.m. to 4:30 p.m. (40 hours per week)

Compensation: $19.74 and $23.54 per hour (wage is determined by total years of experience)
