Home Health Therapist Position for Physical Rehabilitation

LHC Group is a leading provider of post-acute care services, and we are currently seeking a skilled Physical Therapist to join our team in Danville, Kentucky.
About the RoleAs a Physical Therapist with LHC Group, you will have the opportunity to work with patients in their homes, providing personalized rehabilitation services to help them achieve optimal health and independence.
The ideal candidate will have current Physical Therapy licensure in the state of Kentucky and CPR certification. A reliable vehicle and valid driver's license are also required.
This is a full-time position with a competitive salary range of $80,000 - $120,000 per year, commensurate with experience. Our benefits package includes holiday pay, medical, dental, vision, 401k retirement plan, continuing education opportunities, and employee assistance programs.
ResponsibilitiesThe successful candidate will be responsible for assessing and evaluating patient care needs related to functional status/mobility, endurance/stamina, pain, and other medical conditions. They will work closely with the physician to develop a therapy plan of care and provide interventions aimed at improving the patient's well-being.
Evaluating the patient's functional status and physical therapy needs is a key aspect of this role. The candidate will also observe and report any changes in the patient's condition to the supervising nurse and/or physician.
Treating patients to relieve pain, develop and restore function, and maintain maximum functioning using accepted standards of practice is an essential part of this job.
Qualifications and RequirementsWe are looking for a motivated and compassionate individual who possesses excellent communication and interpersonal skills. The ability to work independently and as part of a team is essential for success in this role.
- Current Physical Therapy licensure in the state of Kentucky
- Current CPR certification
- Valid driver's license and reliable vehicle
