Job Title: Home Health Nurse

We are seeking a compassionate and skilled Home Health Nurse to join our team at AllCare Homecare LLC. As a Home Health Nurse, you will provide high-quality nursing care to seniors and individuals with disabilities in their own homes.

Responsibilities:
  • Deliver professional nursing services in a home setting and provide case management for primary patients.
  • Complete initial assessments and plans of care for home care members.
  • Prepare clinical and progress notes, submitting all required paperwork daily.
  • Coordinate services, informing the physician, management, and other personnel of changes in the member's condition and needs.
  • Ensure quality and accuracy of patient records and charts in compliance with Medicare and Medicaid standards.
  • Demonstrate compassion while working with ill, disabled, elderly, and/or emotionally disturbed patients.
  • Respond promptly to telephone calls and emails.
  • Participate in continuing education programs to stay current on home health practices, laws, and regulations.
  • Perform other duties as assigned.
Requirements:
  • RN with current MA license, BSN preferred.
  • One or more years of current nursing experience in certified home care is preferred, but not required.
  • Ability to sit, stand, bend, lift, kneel, climb stairs, and move patients safely as required.
  • Ability to work in the patient's home environment as well as the office.
  • Demonstrate strong verbal, written, and interpersonal skills to communicate effectively.
  • Must have reliable automobile, valid driver's license, and current auto insurance.
  • Fluent in English with skills in a second language a plus.
  • Computer skills sufficient to capture notes and required documentation.
