Job Summary

We are seeking a skilled Patient Care Coordinator to join our team at THRIVE. As a key member of our healthcare team, you will be responsible for assisting residents with daily activities, maintaining a safe and healthy environment, and providing routine individualized healthcare needs.

Key Responsibilities:

  • Assist residents with daily activities, including personal hygiene and daily living tasks
  • Maintain a safe and healthy environment, including reporting any changes in health or safety concerns
  • Provide routine individualized healthcare needs, including vital sign monitoring and recording
  • Discuss observations with leaders and record patient care information

Requirements:

  • High school diploma or equivalent preferred
  • Current State of Georgia Nursing Assistant Certification
  • Previous experience as a CNA is strongly preferred
