Job Summary

We are seeking a highly skilled and compassionate Social Worker/LSW to join our team at Northern Light Health. As a key member of our Care Management department, you will play a vital role in ensuring the smooth transition of patients from our hospital to their homes and communities.

Key Responsibilities
  • Collaborate with interdisciplinary teams to develop and implement comprehensive discharge plans for patients with complex needs.
  • Conduct thorough assessments of patients' social, emotional, and environmental needs to identify potential barriers to discharge.
  • Develop and maintain relationships with community resources and providers to ensure seamless transitions of care.
  • Provide education and support to patients and their families to promote self-advocacy and empowerment.
  • Participate in quality improvement initiatives to enhance patient outcomes and satisfaction.
Requirements
  • Master's degree in Social Work (MSW) or related field.
  • Current licensure as a Licensed Social Worker (LSW) in the state of Maine.
  • Excellent communication and interpersonal skills.
  • Ability to work effectively in a fast-paced environment.
  • Strong analytical and problem-solving skills.
Working Conditions
