POSITION SUMMARY:

A Case Manager is responsible for ensuring continuity of care aimed at achieving permanent placements for children. This role involves a case management approach that includes identifying, developing, and linking community resources to meet the child's needs throughout their journey in the system, while also documenting their progress.


KEY RESPONSIBILITIES:

  • Conduct assessments and formulate treatment plans.
  • Ensure the psycho-social well-being of clients.
  • Meet clinical contract and regulatory compliance objectives.
  • Provide appropriate case management services.
  • Deliver counseling and support to clients and their families.
  • Assist children in completing their education through eligibility assessments, intake processes, evaluations, referrals, transportation, and monitoring attendance.
  • Maintain comprehensive documentation related to counseling services.
  • Record contact notes, assessments, testing results, intake forms, interview notes, correspondence, and statistics.
  • Schedule and conduct client visits at convenient locations.
  • Facilitate informal and formal interactions with individuals relevant to specific clients.
  • Keep in contact with visiting teachers to initiate necessary referrals.

Location: Highlands County

QUALIFICATIONS:

Education & Experience: A Bachelor's Degree with a minimum of 4 years of post-degree experience in human services, preferably in counseling or case management; or a Master's Degree with 0-3 years of post-master's clinical, counseling, or case management experience.


Physical Requirements: Ability to travel regionally on a regular basis and statewide as required; capability to bend, lift, and carry up to 30 pounds.
