Job Summary:
We are seeking a skilled Licensed Practical Nurse to join our dialysis team as a float LPN. In this role, you will be responsible for providing high-quality patient care and working collaboratively with our multidisciplinary team to promote wellness and optimize patient outcomes.
Key Responsibilities:
- Provide direct patient care utilizing our hospital's policies, procedures, and guidelines, as well as standards of practice and clinical pathways.
- Assess and document patient progress toward health goals, modifying the plan of care as needed in collaboration with the RN.
- Collaborate with the healthcare team to identify specific patient needs and initiate appropriate interventions to amend identified variances from the plan of care.
- Recognize and respond to issues that may cause problems, referring issues to the RN as needed and utilizing the escalation policy.
- Verify and document patient advance care directives for assigned patients and maintain open channels of communication through intra and inter shift reporting.
- Process medication orders and refills through our Workflow system, verifying dosage and medication using various tools and progress notes.
- Identify patient satisfaction/dissatisfaction with the plan of care and implement conflict resolution skills to address issues.
Requirements:
- High School or Equivalent Technical Diploma
- Graduate of accredited school of licensed practical nursing
- 0 - 1 year nursing experience working in a hospital, ambulatory, private practice, or home health care setting preferred.
