Fair Haven Community Health Care is seeking a qualified and experienced Intake Therapist to provide crisis emergency care and perform comprehensive psychosocial assessments in a community behavioral health and addiction setting.
Key Responsibilities- Triage clinical emergencies and implement program-specific safety and consultation protocols.
- Complete crisis assessments of adults and/or children who are in crisis, attending to their safety and needs.
- Develop treatment plans with and for the person in crisis, utilizing community-based health, behavioral health, and social services to ameliorate the presenting crisis.
- Complete diagnostic assessments, voluntary outpatient program documentation, and other supporting documentation at first meeting, as well as orient new clients to services offered in the outpatient clinic.
- Work closely with behavioral health patient navigators, the BH Director, and team of clinicians to ensure a smooth transition from enrollment to ongoing treatment.
- Complete required evidence-based screening tools, such as GAD, PHQ-9, and CSSR, CAGE, and social determinants of health (SDOH), among others.
- Collaborate and communicate with the Billing Department regarding medical insurance issues and payment options.
- Bilingual in English and Spanish.
- Master's Degree in a behavioral health and/or addiction field and licensed independent professional in the state of CT (LCSW, LPC, LMFT, or LADC).
- Extensive experience working with culturally diverse urban populations, a wide age range, and a variety of clinical presentations and knowledge of behavioral health, child welfare systems, and community resources.
